Introduction: Suicidal ideation is a marker of mental vulnerability and is a risk factor for suicide and a predictor of completed suicide; can increase up to 30 times the chance of self-elimination. Employment status is one of the areas that most significantly affects the physical and mental health of individuals. Objective: To assess whether there is an association between employment status and suicidal ideation during confinement due to the COVID-19 pandemic in Peruvian adult residents screened for depression in May 2020. Materials and methods: Analytical cross-sectional study, based on a database of the questionnaire "Mental health in the Peruvian population during the COVID-19 pandemic" owned by the Executive Directorate of Mental Health of the Ministry of Health carried out in Peruvian adult residents in May 2020, during the confinement due to the COVID-19 pandemic. Suicidal ideation was measured by item 8 of the PHQ-9 instrument and employment status was constructed from the variables essential occupation and employment status during quarantine. In the descriptive analysis we used absolute and relative frequencies for the categorical variables, mean and standard deviation for numerical variables, and a multivariate analysis using generalized linear models (Poisson family). Results: A total of 22,629 participants were included in the analysis. The prevalence of suicidal ideation was 27.71%, affecting more people aged 18-24, single and residing in Metropolitan Lima. It was observed that those with an essential occupation were 4% less likely to have SI than those with a non-essential occupation (PR=0.96; 95% CI 0.89, 1.03), and the unemployed were 1.06 times more likely to have SI than those with a non-essential occupation. essential (PR=1.06; 95% CI 1.01, 1.12). Previous psychiatric diagnosis, harmful habits, and cardiac and immune diseases are associated with the presence of suicidal ideation. Conclusion: Suicidal ideation is associated with unemployment and not with essential occupation.
