Introduction: Iron deficiency is the most widespread micronutrient deficiency in the world, which often results in iron deficiency anemia. Both entities have negative consequences for health and have been associated with obesity. The literature is controversial regarding the association of these pathologies. Objective: To evaluate if there is an association between iron deficiency and obesity in adults who attended a private clinic in Lima - Peru during the period 2013-2018. Methods: Our study design was an analytical cross-sectional. The participants were adults between 18 and 60 years old of both sexes, who attended an outpatient medical consultation in a private clinic in Lima, Peru; between 2013 and 2018. The main variable of interest was iron deficiency, which was defined by a serum ferritin value <32 mcg/L in men and <10 mcg/L in women. Exposure variables were obesity measured by both body mass index (BMI) and bioelectrical impedance (BI); according to BMI obesity was considered if it was 30 kg/m2 or higher, while according to bioelectrical impedance, it was considered high if in men it was greater than or equal to 25%; or in women greater than or equal to 30%. The generalized linear regression model of the Poisson family was used to calculate the magnitude and direction of our association, and it was measured by the crude and adjusted ratio of proportions. Results: No significant association was found between iron deficiency and obesity. In the multivariate analysis of obesity measured by BMI versus iron deficiency, it was found that in the adjusted model, overweight people have a 10% lower probability of having iron deficiency compared to people with normal weight (OR: 0.90, CI: 0.41 – 1.98), while people with obesity had a 1.17% higher probability of presenting iron deficiency compared to people with normal weight (PR: 1.17, CI: 0.52 – 2.62). On the other hand, according to Bioelectrical Impedance, in the adjusted model it was found that people with obesity had an 18% lower probability of presenting iron deficiency compared to people without obesity (PR: 0.82, CI: 0.32 – 2.08). Conclusions: No statistically significant association was found between iron deficiency and obesity measured by body mass index and bioelectrical impedance.
