Introduction: Out-of-pocket health expenditure (OOPHE) is the extra expense incurred by a family for health services, and if it exceeds 30%, it can lead to catastrophic spending. The OOPHE generated in dental care causes an economic loss that families cannot avoid. Therefore, people opt to receive care in private dental clinics or centers because public insurance does not provide quality oral health care due to the scarcity or lack of different treatments. Objective: To compare the out-of-pocket expenditure in patients attended in dental services affiliated with health insurance registered in the 2020 National Household Survey (ENAHO) during the COVID-19 pandemic. Materials and Methods: An observational, cross-sectional, and analytical study was used. The data recorded by the 2020 National Household Survey (ENAHO) were used for the analysis. Results: The out-of-pocket expenditure in dentistry made by patients affiliated with some health insurance had an average of S/. 226.61 and a median of S/. 80. In the SIS, the OOPHE had a median of S/. 50, and in the EPS, S/. 180. In urban areas, it had a median of S/. 100, and in rural areas, S/. 50. Conclusions: Women generated a higher OOPHE compared to men. Older adults made higher out-of-pocket expenditures compared to other age groups. The out-of-pocket expenditure of people living in urban areas was double that of those living in rural areas.
