Background: Acute appendicitis is a frequent pathology in children, with a challenging diagnosis and a high complication rate. In recent years, a group of predictors obtained from the Complete Blood Count, have been studied due to their easy accessibility and potential utility in the diagnosis of acute complicated appendicitis in this population. Objective: To compare the hematologic predictors in pediatric patients with acute complicated and uncomplicated appendicitis. Material and Methods: Observational, Cross-sectional, Retrospective, Comparative study. We included all the patients younger than 14 years old who were surgically diagnosed with acute appendicitis during the period 2021-2023 in a national hospital in Lima, Peru. The capacity to predict acute complicated appendicitis or uncomplicated appendicitis of the variables Leucocyte count (L), Immature Granulocyte count (IG), Neutrophil-Lymphocyte Ratio (NLR), Platelet-Lymphocyte Ratio (PLR) and Systemic Immune-Inflammation Index (SIII) was analyzed through the application of a Receiver Operating Characteristic (ROC) analysis. Results: The present study included 522 patients from which 85.25% were acute complicated appendicitis. The hematologic predictors SIII, PLR and NLR were statistically significant with Areas Under the Curve (AUC) of 0.667, 0.664 and 0.677 respectively. Summary: The hematologic predictors L, IG, PLR, NLR and SIII are not useful to predict acute complicated appendicitis in patients younger than 14 years old. Additionally, the predictors NLR, PLR, and SIII may be useful to predict acute uncomplicated appendicitis in these patients.
