Objectives: To describe the diagnostic performance of serum tests: FIB-4, APRI, NAFLD score versus liver biopsy in the assessment of advanced fibrosis in obese patients treated at the Avendaño clinic from 2016 to 2018. Materials and methods: This is an observational, cross-sectional, analytical and retrospective study of obese patients evaluated to undergo gastric sleeve surgery. As part of the clinic's standardized protocol, patients also underwent liver biopsy after signing a consent to study liver involvement caused by obesity. In this patient population, the APRI, FIB-4 and NAFLD score tests were calculated and their validity, safety and likelihood ratio were determined. Results: A total of 203 obese patients were evaluated, finding that the mean BMI was 37.99, 41 patients with diabetes mellitus; The mean platelet count was 302.23 U/mm3, the SPT was 53.73 U/L, the SGT was 35.58 U/L, the albumin was 4.38 gr/dL and the glucose was 92.59 mg/dL. 19.7% presented advanced liver fibrosis. The FIB-4 test reported a sensitivity of 2.5%, specificity of 98.39%, positive predictive value (PPV) of 50% and negative predictive value (NPV) of 80.58%. The APRI test reported a sensitivity of 10%, specificity of 98.16%, PPV of 57.14% and NPV of 81.63%. The NAFLD test reported a sensitivity of 10%, specificity of 99.39%, PPV of 80% and NPV of 81.82%. Conclusion: The noninvasive tests APRI, FIB-4 and NAFLD score index are highly specific, but not very sensitive.
