Introduction: Acute appendicitis is the most frequent surgical pathology in the emergency department of a hospital. A rapid and accurate method is necessary to avoid excessive paraclinical examinations and unnecessary appendectomies. Diagnostic scores are systematic processes to order the semiology and laboratory and arrive at the appropriate diagnosis. Objective: To determine the efectivity of the Dolnel and Alvarado Scale Test as a diagnostic test of acute appendicitis in the Hospitals of MINSA and ESSALUD-Cusco, period 20112016. Material and methods: Cross-sectional, analytical, retrospective study of diagnostic validity performed at the Surgery Service at the Regional Hospital of Cusco, Hospital Antonio Lorena and Adolfo Guevara Velasco National Hospital during the period 2011-2016, with a design 316 patients with acute appendicitis who met the inclusion criteria were included, clinical histories were reviewed where the Dolnel Test and Alvarado Scale were applied before being appendectomized. A database was entered and the SPSSV23.00 program compared the diagnostic capacity and proportional area of the ROC curve between the tests and histology. Results: The mean age was 34 years, male predominance was 55.7%. Disease duration was 16 ± 8 hours. The sensitivity of the Dolnel test was 85.3% (for a score> 5 ) And a specificity of 67.4%, the positive predictive value of 94.3% and negative 42%. The sensitivity variable for the Alvarado scale was 78% (for a score 5) and a specificity of 44.2%, positive predictive value of 89.8% and negative of 24%. Finally, an area under the ROC curve for the Dolnel Test was obtained with a cut-off point of 5 was 76.4% and for the Alvarado Scale with a cut-off point in 5 it was 61.1%. Conclusions: The diagnostic efficiency of the Dolnel Test is superior to the diagnostic efficacy of the Alvarado Scale, constituting a valuable complement to the timely diagnosis for acute appendicitis.
