Objective: to conduct a comprehensive and systematic review of the available scientific literature on the phytochemical and pharmacological evidence of Cannabis Sativa L. in patients suffering from chronic pain, covering the period from January 2012 to December 2023. Materials and methods: this study adopts a qualitative approach and an observational design. A search strategy was employed in four databases: Scopus, Biomed Central, PubMed and Scielo. The systematic literature review covered a twelve-year period, examining data from preclinical, observational studies and clinical trials on Cannabis Sativa L. Results: of the forty articles reviewed, phytochemical studies highlighted the predominant presence of phytocannabinoids and terpenes in the genus Cannabis. Compounds extracted from this plant demonstrated diverse pharmacological effects in various manifestations of chronic pain, such as cancer-related, postoperative or post-traumatic, neuropathic, orofacial and head, chronic visceral and musculoskeletal pain. Conclusions: this review highlights that the key components extracted from Cannabis may exhibit diverse pharmacological activities related to chronic pain.
