The following study aimed to: Compare maternal and neonatal complications during pregnancy, childbirth and puerperium in pregnant women of extreme and optimal age, treated in the Hospital Amazonian Yarinacocha, period March-August 2018, non-experimental, quantitative, descriptive research of Comparative design, the population was 348 pregnant women of extreme age and 152 optimal age, the sample was 100 pregnant women of extreme age and 50 optimal age. The technique was documentary review and the instrument data collection sheet. Results, the sociodemographic characteristics were: secondary education 66%, 82%, 80%; Marital status 84%, 78%, 90%; housekeeper occupation 92%, 92%, 88%; Urban origin 92%, 92%, 88%. According to obstetric characteristics, extreme age pregnant women nulliparous adolescents 86% and 98% elderly multiparous, optimal age 68% multiparous, in the type of delivery, vaginal delivery with 58% and 68% for adolescents and optimal age, in Cesarian age was 35 with 56%. Maternal complications were: anemia 34%, 26% and 26%; 22% of elderly women presented severe preeclampsia. During delivery, there was disproportion of the pelvic head with 22% in extreme age and 14% in optimal age, in the puerperium 20% of the adolescents presented perineal tear, 10% of the elderly presented uterine atony. The characteristics of the newborn were, APGAR at 1´ normal (value 8-10) for both groups 96% and 98% in extreme age and 100% in optimal age, there was only mild asphyxiation in extreme age aged 4% and 2% in adolescents. According to APGAR at 5´ both groups was normal, the predominant gestational age was 38-39 sem. for both groups, the weight for both groups of 3000 - 3900 g., weight <3000g in extreme age with 8% for adolescents and 10% for elderly women; for the weight of 4000 g. in extreme age with 8% elderly and 2% adolescents. Neonatal complications were low birth weight in extreme age with 8% and 10%, in optimal age only 2%, these complications are mostly in elderly women with 40%. In conclusion, the maternal complication was anemia in adolescents in 34%, in elderly ITU in 28%, 26% in elderly women with hypertensive syndrome. In childbirth the complication was pelvic head disproportion in both groups with 22% in extreme age and 14% in optimal. Neonatal complications were in elderly women with 30%, low birth weight predominated in 10%.
