The following thesis, titled as, "SYSTEM OF INFORMATION BASED ON THE AUTOCONCEPT TEST FORM 5 FOR THE ASSESSMENT PROCESS OF SELF-ESTEEM IN THE STUDENTS OF THE SECUNDARY LEVEL IN 0775 OF THE HUIMBAYOC DISTRICT, PROVINCE OF SAN MARTÍN", it has as general objective, to improve the evaluation process of self-esteem with the implementation of an Information System based on the AF5 test. To achieve this objective, this was proposed to describe the activities of the implementation of the Information System, in order to determine the results of the self-esteem in the evaluation process, and thus to contrast the influence of the information system before and after implementation in the evaluation of self-esteem. For the development of the information system, the psychological evaluation process of the AF5 test was taken as a basis, as well as different methodologies for the analysis, prototype and construction of the system. It was worked with 148 students, and as result of the implementation of the information system, the process of the evaluation of self-esteem was improved, with which I can conclude that the average of the differences was significant at 5%, with a value of the test t calculated of -52,84 with 147 degrees of freedom, taking the decision to reject the null hypothesis. That is, for a level of significance of 0.05, the value of t is -1.655, with 147 degrees of freedom. It also reduced the times of the evaluation processes of self-esteem by 68.7%.
