Appendicitis is one of the most frequent emergencies seen in hospitals around the world; therefore, its early diagnosis and immediate appendectomy with an adequate surgical technique prevent the appearance of complications. General Objective: To identify the factors that delay the surgical treatment of acute appendicitis in patients treated at the Minsa II – 2, Tarapoto Hospital from July - December 2017. Materials and Methods: Cross-sectional design, the sample consisted of 155 patients who met the inclusion criteria. Data collection card was used and the analysis was carried out in the SPSS 24 program. Results: 155 patients were studied, 27.1% were between the ages of 14 - 20 years, 52.3% were female, 51% lived in rural areas and 51% were complicated acute appendicitis according to pathological anatomy. The factors that delay appendectomy are self-medication (p = 0.013) with predominance of NSAIDs 75.9% and out-of-hospital delay (p = 0.000) with an average of 28. 72 hours with a predominant interval of 12 - 24 hours. No significance was observed regarding distant geographic location, error in diagnosis, and in-hospital delay (p = 0.720, p = 0.078 and p = 0.309), where only 1.9% were more than 6 hours from the Hospital, 0.6% I present error in the diagnosis and 69% admission to the operating room after 6 hours from admission to emergency. Conclusion: the factors associated with the delay in the surgical treatment of acute appendicitis are mainly self-medication and out-of-hospital delay.
