Maternal morbidity is a stage before maternal mortality occurs, so it is known as its precursor; therefore, determining the factors associated with extreme maternal morbidity will help to know which factors can cause a complication that occurs during pregnancy, the puerperium, and even during childbirth. This will prevent admission to intensive care, improve health indicators and reduce costs to the facility. Objective: To determine the factors associated with extreme maternal morbidity in women who were managed in intensive care at Hospital II-2 Tarapoto. Materials and methods: Descriptive case-control study in a population of 138 patients, divided into 69 cases and 69 controls, in which we sought to determine the association between various factors with the presence of extreme maternal morbidity. Results: The associated risk factors were found to be women from rural areas (p:0.01; OR: 2.45; 95%CI: 1.198 - 5.014), gestational age (p:<0.001 OR: 4.68; 95%CI: 2.224 - 9.850), maternal age (p:<0.001 OR: 4.68; 95%CI: 2. 224 - 9.850), antenatal controls (p:<0.001; OR: 4.00; 95%CI: 1.973 - 8.134), the presence of any comorbidity (p: 0.01; OR: 2.683; 95%CI: 1.257 - 5.727), history of caesarean section (p: <0. 001; OR: 8.820; 95%CI: 4.071 - 19.109), hypertensive disorder (p: 0.001; OR: 3.098; 95%CI: 1.547 - 6.202), and organ failure (p:<0.001; OR: 46.439; 95%CI: 6.087 - 354. 238), on the other hand, caesarean section as a route of term gestation is an associated factor but is not identified as a risk factor (p: <0.001, OR: 1.481; 95%CI: 0.727 - 3.02). Conclusions: Risk factors associated with extreme maternal morbidity were found to be women from rural areas, with gestational age less than 37 weeks, with less than 6 birth controls, with some comorbidity, history of caesarean section, with a sensory disorder or organ failure.
