Bacterial resistance to antibiotics is a public health problem and is reported in different health institutions in Peru. The objective of this study was to detect resistance genes to cephalosporins and carbapenems in Klebsiella pneumoniae in isolates from patients from INEN - Peru, during 2018, for this purpose, 34 isolates of K. pneumoniae that were stored at -80 °C were evaluated. These bacterial cultures were reactivated to confirm their identity using the MALDI-TOF mass spectrometry technique, to then detect cephalosporin resistance genes using conventional PCR and carbapenem resistance genes using Gene Xpert Carba-R real-time PCR. As results, resistance genes to cephalosporins were detected, the bla TEM gene in 56% of the bacterial cultures, followed by bla CTX-M in 39% and the bla SHV gene in 27%, in addition resistance genes were detected. to the bla NDM carbapenems in 76%, being the most frequent, and the bla KPC gene the least frequent in 6%. It was concluded that K. pneumoniae subsp. pneumoniae was the subspecies that presented more resistance genes to cephalosporins and carbapenems such as bla TEM, bla CTX-M and bla NDM. In addition, cultures with co-production of cephalosporin resistance genes with carbapenem resistance genes were detected in 67.65%, where 11.76% presented up to 4 resistance genes in the same isolate, such as the bla CTX-M, bla SHV, bla TEM, and bla NDM.
