The main objective of the present research was to determine the influence of self- esteem on treatment adherence in adult patients with HIV from a Hospital in Lima. The research is descriptive correlational type and the sample consisted of 333 patients, including both men and women aged between 18 and 59 years. The instruments used were the Coopersmith Self-Esteem Scale, validated by Lachira, and the Antiretroviral Treatment Adherence Questionnaire. The results revealed an influence of self-esteem on treatment adherence, with an R2 of 0.0716. Additionally, a weak positive and statistically significant association (rs 0.214, p < .001) between self-esteem and treatment adherence was observed. A significant relationship was also found between self-esteem levels and viral load (chi-square = 18, p = 0.035), as well as a significant difference in treatment adherence based on sexual orientation (chi-square = 8.36, p = 0.015). Finally, it was identified that the study participants exhibited high and moderately high levels of self-esteem, as well as a high level of treatment adherence.
