Older adults experience a series of modifications in various bodily systems and changes in overall functionality levels. The decrease or loss of functionality in older adults leads to greater dependence. Pain is also a predictor of disability in this age group, with greater pain correlating with greater loss of general functions. The aim of the study was to determine the relationship between the intensity of lower back pain and functionality in older adults at a clinic in the city of Lima, 2023. A quantitative, observational, and cross-sectional study was conducted. The sample consisted of 90 older adults. The instruments used were the numerical pain scale and the Katz index. The results showed that a higher number of older adults experienced no pain (37.8%). Regarding functionality, a higher number of older adults had moderate disability (43.3%). When crossing the variables of lower back pain intensity and functionality, a p-value of 0.003 was obtained, rejecting the null hypothesis and indicating a statistically significant relationship between the intensity of lower back pain and functionality. It was concluded that a relationship exists between the study variables.
