To evaluate whether the Parkland Scale can determine intraoperative difficulties during laparoscopic cholecystectomy at the Regional Teaching Hospital of Trujillo during the study period from January 2018 to June 2024. Materials and Methods: An observational, analytical, cross-sectional study was conducted with 246 patients undergoing laparoscopic cholecystectomy due to gallstone disease. Clinical, laboratory, and surgical data were collected, including intraoperative difficulties and categorization according to the Parkland Scale. Descriptive and inferential analyses were performed using IBM SPSS Statistics 28 software, applying Pearson's Chi-Square and Student's t-tests to determine significant associations. Results: Of the 246 patients, 59 experienced intraoperative difficulties (23.98%). The group with intraoperative difficulties had a significantly higher leukocyte count (9823.88 ± 3967.55 vs. 7814.89 ± 2662.03; p=0.001) and a longer operative time (114.68 ± 35.51 minutes vs. 87.18 ± 30.63 minutes; p=0.001). Additionally, there was a trend towards a longer hospital stay in this group (2.32 ± 1.04 days vs. 2 ± 1.12 days; p=0.052). The proportion of male patients was higher in the group with intraoperative difficulties (28.81% vs. 17.11%; p=0.050). The distribution according to the Parkland Scale showed that grades IV and V were significantly associated with greater intraoperative difficulties (p=0.001). The most frequent complications were intraoperative bleeding (21 cases), followed by bile decompression (18 cases) and the need for drains (17 cases). Conclusions: The Parkland Scale allows for the determination of intraoperative difficulties during laparoscopic cholecystectomy, with greater difficulties associated with grades IV and V of the scale
