Introduction: Irritable bowel syndrome (IBS) and functional dyspepsia (FD) are the most common functional gastrointestinal disorders (FGIDs). Psychosocial factors may exacerbate symptoms, while physical activity can have positive effects on the gastrointestinal tract. Objective: To determine the association between the level of physical activity and symptoms related to IBS and FD in adults aged 18 to 50 in Lima, Peru, using Rome IV criteria. Methodology: A cross-sectional study was conducted on Facebook users aged 18 to 50 in Metropolitan Lima through a Google Forms survey sent via Facebook from April 2022 to July 2023. Symptoms related to IBS and FD were measured using the Rome IV questionnaire, and physical activity was assessed using the IPAQ tool. Adjusted prevalence ratios were obtained through Poisson Regression. Results: Out of 2151 individuals, 51.98% were male, and the median age was 29 years (IQR: 23-38). 17.0% engaged in low physical activity, and 57.0% in moderate physical activity. 13.48% met the criteria for IBS, and 35.84% for FD. IBS symptoms had a lower prevalence in individuals with high physical activity compared to low activity (PR 0.67; 95% CI 0.49-0.93). Individuals with moderate physical activity had a 20% higher likelihood of FD than those with low activity (PR 1.20; 95% CI 1.01-1.42). Lower prevalence of IBS and FD symptoms was found in working individuals, while high consumption of alcohol and tobacco was associated with an increase in the presence of symptoms. Conclusions: A lower prevalence of IBS symptoms was observed in individuals with higher physical activity, with a significant reduction compared to those with low physical activity. In contrast, individuals with moderate physical activity had a slightly higher prevalence of FD symptoms.
