The objective of this research work was to know the link between healthy lifestyles and the formative evaluation of II.EE sports teams. from UGEL No. 06. The study raises a general problem: What is the relationship between a healthy lifestyle and the 2021 under-12 school sports selection evaluation form? The study is correlational, because the study variables of healthy lifestyles and evaluation were related to the quantitative approach of a non-experimental cross-sectional design. Likewise, this study was carried out with 85 students belonging to children under 12 years of age from UGEL No. 06 in which two questionnaires with a total of 24 questions were used to evaluate the variables. The collected data were processed in the SPSS V26 program, which specializes in descriptive and inferential statistical data analysis. After applying the questionnaires, the relationship between the variables and is significantly related, resulting in a comparison of the general hypothesis: Spearman's Rho correlation coefficient = 0.663** was obtained, which is interpreted at 99.99% ** correlation is significant with two tails at the 0.01 level, interpreting it as a large positive relationship between variables, p = 0.00 (p < 0.01), rejecting the null hypothesis. It was also observed that a healthy lifestyle is directly related to the form of evaluation, i.e. The better the healthy lifestyle, the higher the score, also according to the Spearman correlation of 0.663, it is a high positive correlation that confirms this. There is indeed a relationship between the mentioned variables.
