Objective: This research was interested in identifying clinical, epidemiological, and ultrasound characteristics of thyroid nodules that culminate in thyroid cancer at the Adolfo Guevara Velasco National Hospital in Cusco, 2017 – 2022. Materials and Method: A non-experimental, analytical, case-control, cross-sectional, retrospective and quantitative design was used through the use of medical records. The population was made up of 156 patients distributed in cases (thyroid cancer) and controls (thyroid nodules). Significance was compared using the Chi-square test and Ramdom Tree Forest models were also used for the evaluation of interaction and effect of all variables together. Results: Clinical characteristics (dysphonia OR 2.114 95% CI: 0.609-7.335 and mass palpation OR 1.167 95% CI: 0.621-2.194), epidemiological (family history of thyroid cancer OR 2.0541 95% CI: 0.3651-11.5553 and obesity OR 2.1898 95% CI: 0.9440- 5.1142), ultrasound according to TI-RADS (TR3 OR 0.457 95% CI: 0.2409-0.8684, p < 0.02; TR4 OR 2.545 95% CI: 1.3110-4.9422, p < 0.008) and specific (regular borders OR 0.126 95% CI: 0.048-0.326, p < 0.000006; irregular borders OR 7.914 95% CI: 3.066- 20.43, p < 0.000006; presence of calcifications OR 6.488 95% CI: 2.326-18.09, p < 0.0002 and vascularization OR 3.6 95% CI: 1.599-8.103, p < 0.002) regarding the diagnosis of thyroid nodules culminating in thyroid cancer. And papillary cancer was the most frequent with 98.5% according to pathological anatomy studies. Conclusions: Clinical and epidemiological characteristics are not associated and ultrasound characteristics according to TI-RADS 3 are associated with being a protective factor, on the contrary from TI-RADS 4 to thyroid nodules that culminate in thyroid cancer.
