Abstract: Título: Evaluación de escalas de estratificación de riesgo en pacientes con hemorragia digestiva alta en los Hospitales del Cusco – 2020. Antecedentes: Aguilar S. et al (Lima – Perú, 2015) concluyó que el score AIMS65 es un buen predictor de mortalidad y es útil para predecir la necesidad de transfusión de más de 2 paquetes globulares. Espinoza R. et al (Lima – Perú, 2016) concluyó que el score AIMS65 es un buen predictor de mortalidad y es útil para predecir la necesidad de transfusión de más de 2 paquetes globulares en comparación al score GlasgowBlatchford y el score Rockall. Métodos: Se trata de un estudio cuantitativo de cohorte prospectivo, donde se utilizaron fichas de recolección de datos y para el análisis estadístico el sistema Microsoft Excel 2010 y SPSS versión 22.0. Resultados. De un total de 30 pacientes seleccionados. El sexo masculino fue el de mayor frecuencia siendo 19 pacientes de un total de 30, representando el 63,3%. La edad de presentación más frecuente estuvo entre 31 y 60 años con 17 pacientes (56,7%). La ulcera gástrica fue la etiología más frecuente (50%). La tasa de mortalidad fue de 16%, y del resangrado del 30%. El área bajo la curva fue de 0,944 para Rockall (IC 0,861 – 1,00) (p < 0,05), para AIMS65 fue de 0,980 (IC 0,932 – 1,00) (p<0,05) y para Blatchford Glasgow fue de 0,868 (IC 0,738 – 0,998) (p>0,05) con respecto a la mortalidad. Con respecto al resangrado fue de 0,931 para el AIMS65. Para la estancia hospitalaria fue de 0,624 y para transfusión de 0,762. Conclusiones: El score AIMS65 es similar en cuanto a la alta capacidad de predicción de mortalidad y resangrado que las escalas Rockall y Blatchford Glasgow, y demuestra utilidad para predecir estancia hospitalaria prolongada y necesidad de transfusión sanguínea.

Methods: This is a quantitative prospective cohort study, where data collection sheets were used and for statistical analysis the Microsoft Excel 2010 system and SPSS version 22.0. Results: From a total of 30 selected patients. The male sex was the most frequent, being 19 patients out of a total of 30, representing 63.3%. The most frequent age of presentation was between 31 and 60 years with 17 patients (56.7%). Gastric ulcer was the most frequent etiology (50%). The mortality rate was 16%, and the rebleeding rate was 30%. The area under the curve was 0.944 for Rockall (CI 0.861 - 1.00) (p <0.05), for AIMS65 it was 0.980 (CI 0.932 - 1.00) (p <0.05) and for Blatchford Glasgow was 0.868 (CI 0.738 - 0.988) (p> 0.05) with respect to mortality. Regarding rebleeding, it was 0.931 for AIMS65. For the hospital stay it was 0.624 and for transfusion 0.762. Conclusions: The AIMS65 score is similar in terms of the high predictive capacity of mortality and rebleeding as the Rockall and Blatchford Glasgow scales, and demonstrates its utility in predicting prolonged hospital stay and need for blood transfusion.
