Abstract: RESUMEN En el Capitulo Primero, se habla del Consorcio C&T. Transportistas Asociados S.A. donde se han originado muchos eventos en la carretera parte externa Cajamarca–Yanacocha, tales como: accidentes con lesiones graves y leves, incidentes, daños materiales, actos y condiciones subestandar; la mayor parte de los eventos se ocultaron, por ende no llegaron a determinarse las causas que los provocaron. Muchos accidentes se han producido dentro de las operaciones del cliente principal para quienes realizan servicios de transporte de personal, que fueron reportados. No se ha logrado realizar una eficiente investigación, venian repitiéndose los mismos eventos, por ende los controles no fueron efectivos. La presente investigación tiene su justificación tanto teórica, práctica, valorativa y academica, teniendo en cuenta la importancia en arras del resguardo de la integridad del trabajador, para crear una cultura de seguridad en toda la organización y que todos aquellos investigadores, estudiantes e interesados en el tema puedan sacar provecho, o por el contrario puedan agregar algo a lo realizado. Se ha enfrentado limitaciones como información escasa, desconocimiento de los involucrados y otros. El objetivo principal es lograr un diagnostico adecuado y que éste pueda controlar y minimizar las perdidas no sin antes hacer una valoración del beneficio/costo que ofrece éste diseño al implementarlo. En el Capitulo Segundo, se desarrolla detalladamente todos los contenidos o conceptos teóricos o academicos más utilizados dentro de las bases teóricas y términos más usados, teniendo en consideración que pueden servir de ayuda para comprender el desarrollo de la presente investigación. Luego se desarrolla el planteamiento de la hipótesis respectiva para luego después de analizar se opta por la aceptación o el rechazo. En el Capitulo Tercero, se desarrolla la Metodología de la investigación, donde la operacionalizacion de las variables respectivas se realiza tanto de la dependiente como de la independiente, definiendo así su concepto, sus dimensiones como sus respectivos indicadores; asi también el desarrollo del diseño de la investigación el cual después de analizar, se determina que es No experimental, porque la manipulación de la variable independiente no tiene incidencia directa sobre la variable dependiente implicada en la realidad problemática, es decir es un estudio observacional y descriptivo al aplicar y confrontar la teoría con la realidad. En el Capitulo Cuarto, nos abocamos a los resultados de la empresa, se realiza un diagnostico situacional y organicional de la política empleada, de los lineamientos cumplidos, no cumplidos y en proceso acorde con las normas vigentes, diagnostico mediante índices y porcentajes. Según lo hallado arribamos al desarrollo del diseño propuesto como: políticas, reglamentos, procedimientos, guias, modelos, pautas, controles y planes como el de respuesta a emergencias y por ultimo un modelo de seguimiento y mejora continua. Finalmente se entabla una discusión, se arriba a algunas conclusiones y se propone algunas recomendaciones sobre el proceso de reportes e investigación de accidentes e incidentes, para todos aquellos que estimen implementar y aplicar tal propuesta de diseño.

ABSTRACT In the First Chapter, we talk about the S & T Consortium. Transportistas Asociados S.A. where many events have originated in the Cajamarca-Yanacocha road outside, such as: accidents with serious and minor injuries, incidents, material damages, acts and conditions sub-standard; most of the events were hidden, so the causes that caused them were not determined. Many accidents have occurred within the main client's operations for those carrying out personnel transportation services, which were reported. It was not possible to carry out an efficient investigation, they were repeating the same events, and therefore the controls were not effective. This research has its theoretical, practical, evaluative and academic justification, taking into account the importance of safeguarding the integrity of the worker, to create a culture of security throughout the organization and that all those researchers, students and interested in the topic can benefit, or on the contrary can add something to what has been done. It has faced limitations such as poor information, unawareness of those involved and others. The main objective is to achieve an adequate diagnosis and that it can control and minimize losses, without first making an assessment of the benefit / cost that this design offers when implementing it. In Chapter Two, it is developed in detail all the contents or theoretical or academic concepts most used within the theoretical bases and most used terms, taking into consideration that they can help to understand the development of this research. Then the approach of the respective hypothesis is developed so after analyzing it is opted for acceptance or rejection. In Chapter Three, the Research Methodology is developed, where the operationalization of the respective variables is performed by both the dependent and the independent, thus defining its concept, its dimensions as its respective indicators; as well as the development of the research design which, after analyzing, is determined to be non-experimental, because the manipulation of the independent variable has no direct incidence on the dependent variable involved in the problematic reality, that is, it is an observational and descriptive in applying and confronting theory with reality. In Chapter Four, we focus on the results of the company, a situational and organizational diagnosis of the policy used, the guidelines complied with, not complied with and in accordance with current norms, diagnosis through indexes and percentages. According to what we found, we arrived at the development of the proposed design as: policies, regulations, procedures, guides, models, guidelines, controls and plans such as emergency response and lastly a continuous monitoring and improvement model. Finally, a discussion is made, some conclusions are reached and some recommendations are proposed on the process of reports and investigation of accidents and incidents, for all those who consider implementing and applying such design proposal.
