The present comparative study analyzed the impact of affiliation with the Comprehensive Health Insurance (SIS) on out-of-pocket spending for families with chronic illnesses in the regions of Costa, Sierra, and Selva in Peru. Through a quantitative approach, it was demonstrated that SIS affiliation is associated with a significant reduction in out-of-pocket spending, being most pronounced in Costa, followed by Sierra and Selva. These findings highlight the importance of SIS coverage as an effective measure to address the economic challenges related to chronic illnesses in different regional contexts of the country. Additionally, a greater economic burden was identified in rural areas, especially in the Selva region, underscoring the need to improve access to healthcare services in remote areas. These conclusions point to the importance of strengthening universal health coverage policies and implementing specific strategies to ensure equitable access to medical care across the Peruvian territory, thus contributing to the improvement of health and well-being of the population.
