Abstract: Los leiomiomas son tumores mesenquimales benignos de
infrecuente localización en el estómago, se presentan durante la quinta y
sétima década de vida. Deben diferenciarse de lesiones benignas como
schwannoma, y lesiones con potencial maligno como los tumores de estroma
gastrointestinal (GIST).
Presentación del caso: Varón de 20 años con historia de pirosis y
epigastralgia de 4 años de evolución, que consultó por presentar episodio de
melena, hematemesis y epigastralgia. La tomografía abdominal evidenció una
tumoración entre la pared del fondo gástrico y pared posterior del cuerpo
adyacente. La endoscopía mostró una lesión a nivel subcardial de aspecto
submucoso de 30x20mm con estigmas de sangrado reciente. Se realizó una
resección laparoscópica. El resultado del perfil inmunohistoquímico descartó el
diagnóstico de GIST, debido a que presentó positividad difusa para desmina y
negatividad para los marcadores CD117, CD34 Y S100. El paciente evolucionó
favorablemente y fue dado de alta sin mayor complicación 6 días después de la
cirugía. A los 2 años de seguimiento no ha presentado complicaciones ni
recurrencias.
Conclusiones: En pacientes jóvenes con presencia de síntomas como
epigastralgia crónica se debe considerar la posibilidad de una tumoración en el
estómago, para cuya confirmación diagnóstica es necesario ampliar el estudio
con imágenes y la inmunohistoquímica. La resección quirúrgica es la opción de
tratamiento definitiva.
Leiomyomas are benign mesenchymal tumors which presence is rare in the stomach. During the fifth and seventh decades of life, it must differentiate from benign lesions such as schwannoma and lesions with malignant potential such as gastrointestinal stromal tumors (GIST). Case presentation: A 20-year-old man with a 4-year history of heartburn and epigastralgia, melena, hematemesis, and epigastralgia. Abdominal tomography revealed a tumor between the gastric fundus wall and the posterior wall of the adjacent body. Endoscopy revealed a 30x20mm submucosal lesion at the subcardial level with signs of recent bleeding. The treatment was Laparoscopic resection. The immunohistochemical profile ruled out the GIST diagnosis and confirmed that this lesion was a leiomyoma because it presented diffuse positivity for desmin and negativity for the markers CD117, CD 34, and S100. The patient evolved favorably after surgery and was discharged without major complications six days later. After two years of follow-up, there were no complications or recurrences. Conclusions: In young patients with chronic epigastric pain, a tumor should be considered. So on, it is necessary to extend the study with images. Immunohistochemistry is essential to confirm the diagnosis. Surgical resection is the standard treatment option
