To evaluate and compare the efficacy and safety of antibiotic monotherapy compared to multiple antibiotic therapy in the treatment of perforated appendicitis in pediatric patients, through a systematic review and meta-analysis of clinical trials. Methods: A systematic review and meta-analysis of randomized clinical trials were conducted. Inclusion criteria included articles published from the year 2000 to December 2023, in databases such as PubMed, Scopus, Embase, and Web of Science. The risk of bias of the included studies was assessed, and a meta- analysis was performed to evaluate the efficacy and safety of both treatments. Results: Four randomized clinical trials that met the inclusion criteria were identified. These studies, conducted in various countries, included a total of 432 pediatric patients. It was found that antibiotic monotherapy significantly reduced the incidence of intra-abdominal abscess compared to multiple antibiotic therapy (RR = 0.24, 95% CI [0.10 - 0.55]). However, there were no significant differences in the incidence of surgical site infection, adverse effects, or length of hospital stay between the two treatments. Conclusion: Antibiotic monotherapy appears to be an effective and safe option in the treatment of perforated acute appendicitis in pediatric patients, especially due to its ability to reduce the incidence of intra-abdominal abscess. However, further studies are needed to confirm these findings and establish clear treatment guidelines.
