Objective: To determine the risk factors for Acute Respiratory Distress Syndrome in patients with multisystem trauma at the Hospital Regional Docente de Trujillo from January 2010 to December 2015.. Material and Method: We performed an observational, analytical, case-control study that evaluated 136 patients ≥ 18 years of age with multisystem trauma, who were divided into two groups, 34 patients with ARDS and controls 102 patients without ARDS. Results: The predominant type of trauma was the closed cases in the cases and controls were 70.59% and 58.82% respectively (p> 0.05). In relation to RTS, the mean number of patients in the group of cases was 6.39 ± 0.89 and the patients in the control group were 7.39 ± 0.44 (p <0.001). The risk factors identified for the development of ARDS were those of IOT [91.18% and 66.67% (p <0.001), OR = 5.17; 95% CI (1.47-18.12)]; Mechanical ventilation [82.35% and 53.92% (p <0.01); OR = 3.99; 95% CI (1.52-10.46)]; Blood transfusions [61.76% and 30.39% (p <0.001), OR = 3.70; 95% CI (1.65-8.32)]; Sepsis and pulmonary infections [35.29% and 26.47% vs. 17.65% and 11.76% (p <0.05), (p <0.05); OR 2.55; 95% CI (1.07 - 6.07); OR = 2.70; 95% CI (1.02 - 7.13)]. Conclusions: Orotracheal intubation, mechanical ventilation, blood transfusions, sepsis and lung infections were identified as risk factors for the development of acute respiratory distress syndrome.
