Demonstrate the usefulness of the neutrophil/lymphocyte ratio as a diagnostic predictor of spontaneous bacterial peritonitis in patients with liver cirrhosis at Hospital Belén of Trujillo. MATERIAL AND METHODS: This study is of an analytical, observational, cross-sectional, diagnostic tests. The population consisted of 87 patients diagnosed with decompensated hepatic cirrhosis hospitalized for ascites in the gastroenterology service from Hospital Belen of Trujillo, who met the inclusion criteria. The “sensitivity, specificity, positive and negative predictive value, positive and negative likelihood ratios” were calculated and the “optimal cut off point” of the neutrophil/lymphocyte ratio was determined using the ROC curve and Youden's formula. RESULTS: The results of the “sensitivity, specificity, positive and negative predictive value, positive and negative likelihood ratios of the neutrophil/lymphocyte ratio” were 88.89%, 50.98%, 56.14% and 86.67% 1.82 and 0.22, respectively. The “optimal cut off point” for the neutrophil/lymphocyte ratio was 3.5. Patients with a younger average age and those with a higher score on the Meld-Na scale were associated with spontaneous bacterial peritonitis (p<0.05). CONCLUSION: The neutrophil/lymphocyte ratio is not useful as a diagnostic predictor of spontaneous bacterial peritonitis
